🚨 “AI Hallucinations Are Out of Control!” — Really?

That was the tone of a reporter this morning. Almost every sentence ended in an exclamation mark. You could feel the indignation — How dare AI say something with such confidence and yet be… wrong!?

But here’s the thing: AI isn’t a truth engine. It’s a prediction engine. It doesn’t “know” facts. It’s not coded line-by-line like legacy systems. It’s trained to predict the most statistically likely next word, based on vast patterns in its training data.

So what should we do about hallucinations? 👉 Trust but validate. Always.

Let’s Experiment 🔬

Let’s run a simple experiment to show how prompting can evolve from blind trust to responsible interrogation:

Prompt 1:

“What’s the rocket type that has the best chance of making a manned flight to Mars?”

AI gives you a confident, polished answer. It even feels smart. But…

Prompt 2:

“Summarize the decision-making logic used in this recommendation. Include data sources, assumptions, and alternatives considered.”

Now you’re forcing transparency. Asking: Why this answer? Based on what? What else was ruled out?

Prompt 3:

“How much hallucination do you think is in your responses and explain why?”

This is where AI begins to self-interrogate. It knows it might be wrong. It might even tell you where.

Prompt 4:

“Respond again to the Mars rocket question. This time add rigor — include citations for every claim, replace assumptions with conditional logic, and highlight speculative areas. Estimate any hidden hallucination risk.”

Now we’re getting somewhere. This isn’t just prompting. It’s prompt governance: guard railing.

🛡️ Our Governance Prompt Pack

Our AI Governance Pack now includes:

  • Prompts for audit trails, risk classification, compliance checks
  • Explainability prompts for board reporting
  • Self-diagnostic prompts for hallucination risk
  • A full “prompt review” checklist

These prompts are built for enterprise-grade AI usage.
